The Motor Vehicle Trust Registration Form is a document that legally transfers title of vehicles into the name of the trust. Once the information is received, our team will begin preparing the completed paperwork for you. Occasionally our team may need to reach out to clarify or ask for missing information in order to finalize your paperwork.

Quick Instructions

• When filling out the form, any field that does not apply, please type “N/A”.
• Odometer – Please enter the approximate mileage as of today. (When the vehicle is placed into the trust, not the mileage at original purchase.)
• Purchase Price – Enter the original price you paid when you first bought the vehicle.
• Place of Purchase – For private sales, write “Private Seller” in the blank. The street address is optional for private sales, however, you must provide at least the City and State.
• Uploading supporting documents is optional.
• When you’re done, click Submit.
